It is currently possible to cause an editing conflict within an Action Item even if only a single user is modifying the Action Item.  Steps to reproduce:

1. Open Action Item for editing
2. Go to “Email Log” tab
3. Select an email in the log that has an attachment with it
4. Right click on the attachment in the preview pane and select “Save As…”
5. Navigate to where you intend to save the item
6. At the bottom of the “Save As” dialog box, check the “Add to Project Item” box and select the Action Item that you are in (i.e. the one you opened in step 1 above)
7. Click the “Save” button.  You will be notified that an item has been added to the Action Item that you selected.
8. Make other changes to the Action Item that you have open (again, the one you opened in step 1)
9. Click the “OK” button
10. You will be notified that someone (you) has modified the Action Item, make note of your changes, close the item and reopen to add the changes back in.

This seems like something that should be fixed as it can be particularly bothersome if you have made extensive changes to the Action Item and have to remake the changes due to the locking issue.

That’s a good catch. We’re currently looking into this. To work around this problem, we recommend that you don’t use the Add to Project Item option while you’re editing an Action Item. Instead, in the Action Items page you can select the Action Item in the list, then save the attachment from the preview in the email log section.

I’ve just confirmed that this is still an issue and is reliably reproducible on our end.  I will submit a support ticket shortly.
