In Fourth Edition, the logos were removed as they have been a cause for the email notifications to be blocked as spam. Note that changing the formatting of the forms to add the logos could prevent some users from receiving the form.

To edit the form, follow these steps:

1. Locate the form and open it in Infopath - right click the ,xsn file, and select Design.
2. In the top of the form is the logo section. Right click the whole section and select properties.
3. The Section Properties window will open.
4. On the Display tab, click Conditional Formatting
5. Remove the script for “ForEmailClient is present”.
6. Save the form - click File > Save As Source
7. Publish the form.

The logo will now be visible.
